About the Role
We are seeking an experienced and motivated Social Care Assessor to join our Hospital Discharge Team on a project, supporting adults across Devon through the Discharge to Assess (D2A) pathway.
This is a hybrid working role, offering the flexibility to work from home. The successful candidate will be expected to undertake face-to-face visits as required to complete assessments, reviews, and discharge planning activity.
The role focuses primarily on Pathway 2 and Pathway 3 clients, including the backlog and breach cases.
Key Responsibilities
- Manage a caseload of complex hospital discharge referrals under the Discharge to Assess (D2A) model.
- Complete Care Act 2014 Assessments using a proportionate and strengths-based approach.
- Undertake assessments and reviews for individuals on Pathway 2 and Pathway 3 discharge routes.
- Work proactively to reduce delays and support the resolution of backlog and breach cases across Devon.
- Complete Mental Capacity Assessments in accordance with the Mental Capacity Act 2005.
- Participate in Best Interests Meetings where individuals lack capacity to make specific decisions.
- Conduct Section 42 Safeguarding Enquiries and contribute to safeguarding investigations and interventions.
- Participate in NHS Continuing Healthcare (CHC) Decision Support Tool (DST) assessments and Joint Funding Assessments.
- Liaise effectively with community health teams, care providers, families, advocates, and partner agencies.
- Develop outcome-focused support plans that promote independence, wellbeing, and least restrictive options.
Essential Skills and Experience
- Experience working within Adult Social Care
- knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.
- Experience completing Care Act Assessments and support planning.
- Understanding of the Discharge to Assess (D2A) model and hospital discharge pathways.
- Experience undertaking Mental Capacity Assessments and Best Interests decision-making.
- Experience completing or contributing to Section 42 safeguarding enquiries.
- Full UK driving licence and access to transport to travel throughout Devon.
