We are excited to offer this Social worker opportunity within the Adult Disability Focus side of our Dawlish and Teignmouth Community Health and Social Care team. Here in Devon, we are passionate about developing people and committed to achieving excellence in practice, based on what matters to people.
Our Coastal Team is part of the wider South Devon teams neighbouring with Newton Abbot, and Moor to Sea Health and Social Care Teams. We have a diverse demographic and the team supports work with adults of working age with a physical, lifelong conditions and/ or learning disability including transition to adulthood, as well as older persons with physical disabilities and/ or mental health, including dementia, and carers.
We are currently based in Teignmouth Hospital and cover a large geographical patch. The team is busy and work is fast paced and you will work alongside other social workers, social care assessors and occupation therapists as well as other local health colleagues.

There are well established links with many of the local providers and community services. The Care Act is key to much of the work we do. However we look to support and enable people to realise and achieve their own goals in a wide variety of ways including utilising universal and voluntary services through to commissioned care. Your work in the main will be with people of working age and often disability focused.
You will need to have a strength based, person centred approach in all of your work. As a social worker you will be asked to complete Care Act Assessments and subsequent Care Plans, Mental Capacity Act Assessments, Best Interests Decisions, Court of Protection applications and proceedings and Safeguarding work all of which may result in court work. Visiting clients in their own home/placement is an essential part of this work and as such being able to travel easily within the community is essential.
The nature of this work requires you to be self-motivated, highly organised with great recording skills and in turn you will receive excellent support in a busy and friendly team.
