The Exmouth, Woodbury and Budleigh Health and Social Care Team, part of Adult Social Care, has vacancies for a Social Care Assessor based in Exmouth, covering Exmouth, Woodbury and Budleigh .

You will be working in a community Health and Social care team to offer advice and complete assessments for younger adults and older people, primarily through face-to-face contacts within people’s own homes or within care settings. You will be supporting people to retain their independence, undertaking holistic assessments and support planning, working in partnership with other agencies, making onward referrals, and reviewing services.
We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a social care or health environment and have a good understanding of human needs. Ideally you will also have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team. You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service, and to achieving positive outcomes for vulnerable people.
You will be part of a friendly, supportive and experienced team with regular supervision, appraisal, regular team meetings, and access to learning and development opportunities. The team is made up of Social Workers, Occupational Therapists and Social Care Assessors. We work closely with health colleagues and other professionals to ensure that we are delivering the best possible outcomes to the people of Devon. The Social Care Assessor role within the community team will be working a 5 day week; Monday to Friday, office hours.
