The Moor to Sea Community Health and Social Care Team is seeking permanent Social Workers to join us as we introduce an innovative new way of working designed to support both high-quality practice and professional development.
Whether you are an experienced community practitioner or looking to transition into community-based social work, this is an excellent opportunity to develop your skills within a supportive and forward-thinking team.
As part of our new operating model, we are introducing rotational workstreams that will allow practitioners to undertake focused work in key areas of adult social care. This approach enables team members to build expertise in areas where they already have strengths and interests while also gaining experience and developing confidence across a wider range of practice.

- Care Act Assessments
- Reviews and Reassessments
- Safeguarding
- Discharge to Assess and Intermediate Care
This model supports reflective practice, creates opportunities for learning and specialisation, and helps ensure practitioners have the time and support needed to deliver high-quality, person-centred outcomes for the people we serve.
About the Team
The Moor to Sea Community Health and Social Care Team is committed to supporting people to live well, safely and independently within their own homes and communities for as long as possible. Our practice is underpinned by Devon's Living Well approach, focusing on people's strengths, relationships and community resources to help them achieve the outcomes that matter most to them.
We take a preventative approach to our work, seeking opportunities to prevent, reduce and delay the need for formal care and support wherever possible. We are continually developing our services to ensure people receive the right support, at the right time, from the right professional.
The team consists of experienced Social Workers, Social Care Assessors and Occupational Therapists who bring a wealth of knowledge, skills and expertise. We pride ourselves on being a supportive and collaborative team where colleagues are encouraged to learn from one another and where new team members receive a warm welcome and ongoing support to develop their confidence and practice.

We are an integrated service that works closely with colleagues across the NHS, Short Term Services, the voluntary and community sector, and other community-based partners. About the Role
This is a rewarding and varied role, supporting people across a wide range of settings and circumstances. You will work with young people transitioning into adult social care, adults with disabilities and long-term health conditions, and older adults who wish to remain living independently at home or who require support to recover and rehabilitate following illness, injury or hospital admission.
The role offers excellent opportunities to develop your skills and experience in working with adults who have complex needs, including mental health needs, substance misuse, homelessness, self-neglect and other situations involving significant risk and complexity. It requires professional judgement, empathy and a strong commitment to promoting equality, independence and people's connection to their local communities.
You will work within a range of practice models, including strengths-based assessment and review, the Care Act framework, Discharge to Assess and Intermediate Care pathways. Working alongside multidisciplinary colleagues from health and social care services, you will support people to identify their strengths, build on existing support networks and achieve the outcomes that matter most to them. Using a preventative and asset-based approach, you will help reduce, delay and, where possible, prevent the need for ongoing formal care and support.
As part of our rotational workstream model, you will have the opportunity to develop specialist skills across key areas of adult social care, including assessments, reviews, safeguarding, hospital discharge and intermediate care. You will also gain valuable experience in triaging referrals, risk assessment and responding to safeguarding concerns.
The role includes completing person-centred Care Act Assessments, strengths-based support plans and reviews. You will coordinate care and support arrangements, undertake Mental Capacity Assessments where appropriate, contribute to best interest decision-making, and support safeguarding enquiries to ensure people remain safe while maintaining choice and control over their lives.

You will bring to the role;
- Experience in working with people in a health or social care environment
- Excellent verbal and written communication skills, with the ability to complete high quality assessments.
- The ability to work both independently and within a team.
- The nature of this work requires you to be highly organised with strong recording skills;
- The ability to prioritise referrals and manage competing demands simultaneously.

Social Worker Career Pathway
We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway.
This is a fantastic opportunity to provide you with an achievable, clear pathway for your career; to enable you as a practitioner to reflect and identify your own practice and development on the PCF pathway.
If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).
With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
