Social Worker - Ivybridge/Kingsbridge CHSCT

An exciting opportunity has become available within the Ivybridge/Kingsbridge Community Health and Social Care Team.
This is 12 month project in partnership with health. The focus will be to complete social functions for those who have had a stroke. The expectation will be to work with health professionals to support the journey from hospital back into the community.
The practitioner will need to consider short and long term intervention to support the person to identify their new baseline to independence.
As this is a new role there will be a need to collect statistical data through the use of systems such as Power BI, with the support of the line manager.
