An opportunity has arisen for a social care assessor to join the Tiverton and Cullompton Community Health and Social Care Team on a permanent full time basis 37 hours per week.
The post will be based within the Tiverton and Cullompton team and would suit an experienced social care assessor looking for a fast paced and varied role that will include the completion of strengths based assessments, co-ordination of care, placements and ongoing social care support.
You will be required to complete mental capacity assessments and raise safeguarding adult concerns where necessary. The role would provide an excellent opportunity for anyone who is looking to progress their social care career. You will work closely within an integrated Health team and be required to offer support and guidance to the wider locality. You will develop good working links with voluntary groups in the area and GP surgeries. The nature of this work requires you to be highly organised with strong recording skills and in turn you will receive excellent support in a busy and friendly team.
