An exciting opportunity as arisen in the Devon Onward Care team and the South Hams And West Devon Community Social Care Team. You can be at the forefront of supporting people to return to the community with complex care needs. You will be responsible for facilitating complex discharges from Derriford Hospital and reviewing the service users care needs within the South Hams and West Devon communities once they have been discharge into their community care setting.
We have adopted the Discharge to Assess Model which is facilitating complex discharges from the hospital. You will be based at Derriford hospital and in the community and will be collaboratively with our hospital and community-based colleagues and within a whole integrated system.
The role would provide an excellent opportunity for anyone who is looking to progress their social care career. You will be working alongside a health and social care team which comprises of Discharge Nurses, Senior Social Workers, and other Social Care Assessors and the South Hams and West Devon Community Health and Social Care Team.
This role would suit an individual looking for a fast paced and varied role that will include the completion of strengths based social care assessments, co-ordination of care, placements and ongoing social care support from and following discharge from hospital.
The nature of this work requires you to be highly organised with strong recording skills and in turn you will receive excellent support in a busy and friendly team. You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service, and to achieving positive outcomes for vulnerable people.
You will be fully supported and given training where needed and be fully supported by a friendly, welcoming team.
