The Exeter South Community Health & Social Care Team covers Pinhoe Surgery, Topsham Surgery, Ide Lane Surgery, and the Westbank Practice. It is a multidisciplinary team including a manager, social workers, occupational therapists, and social care assessors.
It is essential that all workers have robust knowledge of the Care Act 2014. Other important legislation includes the Mental Capacity Act, Safeguarding Adults, and the Human Rights Act. Each role requires a practitioner to be confident in their own assessment skills, and able to work independently and as part of a team.
The Social Care Assessor will be required to assess an individual's care needs and identify suitable outcomes. This can involve building upon an individual's own strengths, accessing voluntary support, or commissioning a package of care. Assessments will either be virtual or face-to-face, depending upon the presenting need and risk.
The Exeter South CHSC Team works with all adults over the age of 18, with a physical disability, who require a Care Act Assessment. Referrals are received in equal numbers from all age groups. The earliest intervention required may be attendance at an Education & Health Care Plan (EHCP) review. Preparing For Adulthood (PFA) is an important part of the role. Presenting needs in younger adults is varied. This can range from a Learning Disability to sensory impairment. It is important that workers cultivate a progressive working relationship with local providers. We assess and assist individuals with an Acquired Brain Injury. Social isolation may require us to advocate on an individual's behalf. We do a lot of work with older adults. This can include helping protect individuals who have dementia, Parkinson's disease, Huntington's or Motor Neuron Disease. Much of this work is done alongside our Health colleagues.
