Social Care Assessor (REF 3178)

Moor to Sea Community Health and Social Care Team are looking to recruit two Social Care Assessors to join this friendly and busy team. 

The Social Care Role includes the completion of strengths based Care Act Assessments and Personalised Care Plans.  You also will be supported to complete mental capacity assessments and assist in gathering information relating to safeguarding concerns and will have access to relevant support if needed. 

We work alongside people at different needs including but not limited to;

Older People

People with Primary Mental Health Needs

People with Learning disabilities

People experiencing Substance Misuse

People receiving End of Life Care

Younger Adults

Carers

We work alongside multiple agencies for example the voluntary sector, GP surgeries, Community Therapy and intermediate care teams; community nursing, older person mental health teams and  local councils.

Both posts are permanent and full time but we would be open to part time working for the right candidates.

Moor to Sea whilst based at Totnes Hospital is community facing.  Moor to Sea covers a large predominantly rural area from South Dartmoor, into Torbay and down to Slapton and there will be a requirement to drive within this post. 

You will have access to a comprehensive induction which can include support from a mentor; regular supervision and access to support and training.
