Social Care Assessor
This Social Care Assessor role is a great opportunity for someone who wishes to further develop their skills, knowledge, and experience within the adult Health and Social Care sector. You will receive a comprehensive induction and be supported through regular 1:1 supervision, appraisal, regular team meetings and access to learning and development opportunities.
The successful candidate will demonstrate excellent communication skills and be able to hold their own caseload whilst working as a member of the integrated Health and Social Care team. The applicant needs to be flexible and able to meet the challenges of working in a busy community setting and there will be opportunities to gain experience in areas of special interest within the wider team.
The role will require you to complete care and support needs assessments and develop and review personalised care and support arrangements, in accordance with the Care Act 2014. You will support people to promote their independence and apply a strengths-based approach which seeks to optimise opportunities for people to reduce and delay their need for formal care and support. You will complete mental capacity assessments and raise and triage Safeguarding Concerns. You will support the team by taking a variety of contacts and referrals, provide advice and information, work in partnership with other agencies, and make onward referrals.
We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a health or social care environment and have a good understanding of human needs. You will also have experience of undertaking assessments and working with an eligibility criteria.
The successful candidate will be jointly supported by the Intermediate Care and Therapy Lead and the Team Manager of the Newton Abbot Community Health and Social Care Team.
Due to the large geographic and rural nature of this area you will need to be able to drive and have access to transport.
