A rare opportunity has arisen to join a very supportive, long-established DCC Team, based onsite at RD&E Hospital in Exeter.


Made up of 6 ESWs, 5 SCAs, 1 Coordinator & 1 Team Manager.


Due to the recent promotion/team move of existing SCAs, we have 2 permanent full-time vacancies for Social Care Assessors.


Since the Covid19 Pandemic, our Team function has changed and we now focus primarily on post-discharge case management of STS funded discharges from RD&E Hospital into Care Home placements. These service users are funded via Discharge to Assess for up to 28 days, and require a Care Act Assessment within that time to assess the need for Local Authority support/funding and agree a longer-term care plan. We also need to proactively support their return home, following rehab/recovery, wherever possible. We are also required to play a full part in the NHS Continuing Health Care assessment process, on behalf of the Local Authority, for those service users discharged from RD&E Hospital into Nursing Homes under Pathway 3.


We are also regularly required to move service users from one Care Home to another, for various reasons, with the support of our Arranging Support colleagues, so this SCA role involves significant liaison with the Care Home market - across the whole of Devon & sometimes beyond.


There is good variety in terms of cases, a long-established policy of case self-allocation, and lots of opportunity for learning/development/joint working, as well as professional autonomy. Primarily, though, we are supporting older adults who have complex physical & mental health needs, many of whom are approaching the end stage of their lives.


A significant aspect to this SCA role is the application of & adherence to the Mental Capacity Act (2005), as well as significant liaison with carers/families/friends/advocates, etc. CHDT are often the decision-makers when it comes to Best Interest Decisions.


The successful applicants will get comprehensive support from the rest of the Team, the Coordinator, & from the Team Manager, and will also have a full induction in CHDT.


Caseloads will be carefully managed & well supported, and there will be regular professional supervision (as well as daily ad-hoc advice/support from the Team Manager, where needed).


The successful applicants will also be expected to participate in a daily Duty rota Mon-Fri, and also - once settled in the Team - to work Saturdays on a 2 in 8 basis (1 working, 1 on standby).


CHDT are currently working in a hybrid way, spending some of the time in our office base & some of the time working from home - this can be negotiated, & can be flexible week to week (as covid-safe office space allows). As we are based in an NHS establishment, we are working under NHS rules about maximum number of staff per office to allow for social distancing, etc.


We are also continuing to extend our move back into more face to face working with our service users. Primarily, we are curently working virtually (via MS Teams, etc) but f2f working is increasingly required within Care Homes for some of our caseload (subject to Covid19 outbreaks, etc).
