Job summary
An exciting opportunity to join the LDAP keyworker pilot. The learning Disability and Autism team has been created as part of a key worker pilot designed in partnership with NHS England. This pilot delivers a new way of supporting CYP with Autistic Spectrum Disorders (ASD) and/or a Learning Disability (LD) and their family/carers. The key worker roles will enable CYP and their family/carers to get the right support at the right time and ensure local systems are responsive to meeting their needs in a holistic and joined up way.
The pilot is part of the NHS Long Term Plan which aspires for all CYP with ASD, LD or both, in an inpatient setting, to have a named keyworker by 2024. Devon is one of 13 national key working pilot sites implementing this strategic role.
We are looking for a B7 LDAP Keyworker senior practitioner who has the skills and experience of working with this cohort of YP, who might be interested in joining this innovative and successful team and contribute to its ongoing development.

Main duties of the job
The teams are made up of Senior Practitioners (Band 7) and Assistant Practitioner (Band 4) with business administrative support. The teams work together to share expertise and skills, attend combined training events and share staff supervision.
The teams are trained in Positive Behavioural Support and will be offered Adaptive Mentalisation Based Integrative Therapy. They will link into the capabilities frameworks for working with individuals with ASD and/or LD to ensure they have an adequate skill base and knowledge in these fields. These models will guide them in adopting a team around the key worker model of care, where CYP and their family/carers have a trusted and accessible worker who gains specialist advice and support from all the other professionals involved in their care.
The post holders will work across education, health and social care to enable whole system change to embed principles of transforming care and SEND reforms to improve the experience of CYP and families.
