Are you highly organised, confident with IT systems, and passionate about supporting people in your community? Join Devon County Council as an Arranging Support Co-ordinator and play a key role in helping individuals access the care they need.
As part of our friendly and supportive team, you’ll coordinate the delivery of health and social care services for people with assessed eligible needs. Working closely with care managers, health professionals, and accredited providers, you’ll ensure timely and appropriate support is arranged—whether through commissioned services or direct payments for those choosing to self-direct their care.
What You’ll Be Doing
- Coordinating care arrangements with residential and nursing providers.
- Managing service documentation and ensuring contracts meet legal and policy requirements.
- Coordinating care arrangements for providers delivering personal care and enabling in their own home
- Coordinating care arrangements with supported living provision, live in care or extra-care housing providers.
- Supporting the setup of direct payments for individuals managing their own care.
- Maintaining accurate records and handling sensitive information with care.
- Liaising with professionals and providers via phone, email, and MS Teams.
What We’re Looking For
- Strong communication skills, both written and verbal.
- Confidence using bespoke IT systems and Microsoft Office applications.
- Excellent attention to detail and the ability to manage multiple tasks.
- A good standard of Maths and English (GCSE Grade C/4 or equivalent).
- Experience in a health, social care, or customer-focused environment is desirable.
