This opportunity has arisen for a candidate interested in an Advanced Practitioner role to join our Adult Social Care team specialising in Continuing Healthcare, Funded Nursing Care and partnership working with health colleagues. This post is advertised due to a team member leaving.
This is a specialist role in the Health Partnership Team within Devon County Council. A small team of 3 Advanced Practitioners.
You must be a registered Social Worker, Occupational Therapist or Nurse. You must have experience of working in Adult Social Care and demonstrate excellence in practice in order to support others. A knowledge of the Continuing Healthcare National Framework is desirable, but you will receive support with developing this. It is your Social Care knowledge we need.
We are looking for a dynamic and motivated leader who has a real commitment to improving the care for the people we serve. You should have practice experience in adult social care, with a high degree of emotional intelligence, clarity of thought and curiosity. You will have experience in Multi-disciplinary working and a commitment to supporting colleagues.
You will work alongside another Advanced Practitioner and a small team focused on Continuing Healthcare, health funding streams, joined-up practice and education. You will manage your own time and be accountable for the day-to-day work referred in to the team. The role is varied, working with colleagues across Education, Health and Social Care. The role will require you to advise and support others in high risk cases. You will deliver training to others and be expected to engage with countywide projects and developments. This is an exciting role, which offers variety in a multi-disciplinary context.
You will develop skills to represent Devon County Council in local and regional roles as part of the Local Authority requirements under the National Framework for Continuing Healthcare.

As an Advanced Practitioner you will provide:
Excellent leadership to ensure the delivery of Social Care outcomes through the provision of advice, guidance, support and occasional case management.
Effective communication skills as you work in partnership with statutory and voluntary sector organisations.
You will need to demonstrate that you possess these core competences to underpin achieving excellent practice and performance.
You will have excellent knowledge of health and social care. You will be registered with Social Work England/HCPC as a Social Worker or Occupational Therapist, holding the required qualification and having completed the requirements of post-qualifying learning (for example the Assessed and Supported Year in Employment or preceptorship). You will evidence your experience gained from your previous professional roles and their relevance to this post.
